Health and care partners in Kingston and Richmond win London Personalised Care Award

Health and care partners in Kingston and Richmond have been recognised at the London Personalised Care Awards, hosted by the Coalition for Personalised Care.

The Proactive Anticipatory Care (PAC) team received the Collaborative Working Award for their contributions to supporting personalised care through the PAC service. This was originally launched as a pilot in 2021 and supports local people to be in control of their health and wellbeing.

Under the PAC model, patients with multiple long-term conditions or frailty – who may become reliant on emergency care – are proactively identified. Local GPs, community teams, social care and hospital services then work together to deliver coordinated, personalised support.

The team takes a holistic approach, exploring every aspect of a patient's health and wellbeing. Working closely with patients, families, and carers, they co-create tailored care plans that meet individual needs.

This approach helps people stay healthier for longer, reducing the need for reactive interventions and ensuring local people receive the right care at the right time.

Dr Annette Pautz, Kingston GP, said: "I'm proud to be part of the PAC service, which truly represents the future of the NHS – bringing partners from health, social care and the voluntary sector together to deliver care within local communities and empowering people to take charge of their own health.

"Receiving this award is a fantastic recognition of the collaboration and dedication that have driven this work over the past few years."

Cathy Maker, CEO of Ruils Independent Living Charity, said: "The PAC service has gone from strength to strength, and it's been incredibly rewarding to be able to connect people in our communities with personalised support, helping them to live healthier and more independent lives."

PAC is a collaborative model delivered by partners across health, voluntary and social care, including South West London Integrated Care Board (ICB), Kingston and Richmond general practice, Ruils Independent Living, Staywell, Kingston and Richmond NHS Foundation Trust, Your Healthcare, South West London and St George's Mental Health NHS Trust, and Royal Borough of Kingston upon Thames and London Borough of Richmond upon Thames local authorities.
